Perceived Threats, Errors and Safety in Aerial Work and Low-Capacity Air Transport Operations
11 March 2010The Australian Transport Safety Bureau has released a report that looks into threats and error management by identifying common threats to operations and errors made by pilots in aerial work and low-capacity air transport operations as perceived by flight instructors, check-and-training pilots, chief pilots and line pilots.
The most common threats included external ones such as weather, operational pressure and traffic congestion, and internal threats such as fatigue.
The common identified errors are procedural errors in using checklists, non-compliance with standard operating procedures (SOPs), planning errors, and communication with ATC or other aircraft.
The report also provides ratings of safety deficiencies and offers suggestions on ways to deal with the issues.
